gender affirming care

Baltimore Community Midwives

Baltimore Community Midwives offers a unique blend of comprehensive, personalized, and community-focused care. Opened in 2025 by Tanvi Avasthi, MSN, MA, CNM/WHNP-BC, this practice exists to integrate the principles of Anti-racism, Queer Liberation, and Reproductive Justice to create a space for patients who are usually systematically excluded from accessing quality healthcare. We offer extended 1-hour appointments, allowing for in-depth discussions and building trust between the provider and patient. This approach ensures that each individual receives the attention and care they deserve.

As a practice deeply embedded in the Baltimore community, we offer care that is not only medically rigorous but also structurally competent and community-oriented. We are experienced in gender-affirming care, the Health at Every Size® (HAES®) model, and trauma-informed care. This community-based approach allows us to better understand and address the unique health needs and challenges faced by the greater Baltimore Community as well as the individual’s needs.

By combining the expertise of Certified Nurse Midwives (CNMs) and Certified Professional Midwives (CPMs), we offer a comprehensive range of services that is both unique and necessary. From fertility and conception care, postpartum support, general wellness, and gender-affirming services, our holistic approach ensures that patients receive continuity of care throughout their reproductive journey and beyond.

Tanvi Avasthi, MSN, MA, CNM/WHNP

Tanvi Avasthi (she/her/hers) is a Certified Nurse Midwife, Women's Health Nurse Practitioner, nurse educator, and anti racism organizer. She has worked as a community health nurse, emergency department nurse, and nurse educator prior to working in reproductive health and also holds a Master's in Medical Anthropology.

The throughline of her work has been providing a space for radical care and empathy that centers patients in their contexts. As a midwife, her goal is to encourage and support bodily autonomy. She believes midwives are uniquely primed to support patients' desires to be seen and supported and is excited to support the Baltimore trans community.

She maintains a mighty tropical plant empire and would like to one day go back to beekeeping and teaching yoga.

CPMs are trained and credentialed to offer expert care, education, counseling and support to birthing people during the pregnancy, birth and the postpartum periods.
